Comments:
Licensing Inspector Anderson conducted a unannounced Injury Requiring Outside Medical Treatment (IROMT) inspection in response to a self reported incident received by the licensing office on April 23, 2024. The investigation of the self reported incident was initiated on April 24, 2024 and findings were reviewed on June 10, 2023.

Violation has been cited related to information submitted by the licensee of a report of an injury to a child while under the supervision of a center that requires outside medical treatment of a child in care. Please see Violation Notice for more details.

Violations:
Standard #: 8VAC20-780-40-E
Description: Based on inspection, staff interviews, and record reviews, the Center failed to ensure that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures.

EVIDENCE:
Staff #1 did not follow the center's policy procedures for Level III medical emergencies. Child #1 (age 15 months old) sustained a serious injury requiring immediate medical attention. Per their policy, staff were required to call 911 immediately. Staff did not call 911. The child's parent arrived at the center and called 911 at approximately 11:30 AM.
