Evidence: Staff #1, date of hire 11/18/2020 has not completed new employee orientation training.Plan of Correction: Management will complete the training by Friday of this week.
Standard #: 8VAC20-780-270-A Description: Based on observation and staff interview, areas of the center are not maintained in a clean, safe and operable condition.
Evidence: There was a large hole in the window ledge in the "Green Room". Staff stated that a child put a hole in it this week.Plan of Correction: The hole will be patched.
