Technical Assistance:
Technical assistance was provided and licensing standards were discussed with the director regarding, MAT Training-8VAC20-800-220 and caregiver records 8VAC20-800-200.

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
A renewal inspection was conducted on May 22, 2024, from 11:36am to 12:40pm. There were seven children present and two staff. Records were reviewed for five children and two staff records, along with any requested program records submitted by the provider to determine if required documentation was complete. Children were observed during naptime. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Violations:
Standard #: 8VAC20-800-730-A
Description: Based on observation and interview, the provider did not ensure that medications for children in care shall be stored separately from medications for household members and caregivers.
Evidence:
1. The medication for child #1 was stored in a cabinet in the kitchen with the household member?s and caregiver?s medications.
2. Staff #1 confirmed that the medication for child #1 was stored with the household member?s and caregiver?s medication.

Plan of Correction: Lock box purchased and medicine stored.

Standard #: 8VAC20-800-940-B
Description: Based on observation and interview, the provider did not ensure that the provider shall have an operable thermometer available to monitor refrigerator and freezer compartment temperatures.
Evidence:
1. The provider did not have a thermometer available to monitor the refrigerator and freezer temperatures.
2. Staff # 1 confirmed that there was no thermometer available.

Plan of Correction: Purchased thermometers for freezer and refrigerator.

Standard #: 8VAC20-800-960-E
Description: Based on observation and interview, the provider did not ensure that bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care.
Evidence:
1. Approximately eight bottles of milk and water were stored in the refrigerator and did not contain labels with names or dates.
2. Staff #1 confirmed the bottles were not labeled with a date and the children?s name.

Plan of Correction: No infants bottles it was toddlers cups with water in them but all cups are labeled and dated.

Standard #: 8VAC20-800-60-B
Description: Based on record review and interview, the provider did not ensure that children?s records contain information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation.
Evidence:
1. The record for Child #1 did not contain an allergy action plan for the physician diagnosed food allergy.
2. Staff #1 confirmed there was no documented allergy action plan in the record for child #1.

Plan of Correction: Gave parent action plan to have signed by physician.

Standard #: 8VAC20-800-270-A
Description: Based on observation and interview, the provider did not ensure that firearms of every type and purpose shall be stored unloaded in a locked container, compartment, or cabinet, and apart from ammunition.
Evidence:
1. The firearm in the household was locked together with the ammunition.
2. Staff #1 confirmed that the firearm was locked and stored together.

Plan of Correction: Gun was unloaded and locked in gun case. Locked ammunition in separate cabinet.
