Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on August 31, 2022 from 11:30 a.m. through 1:15 p.m. There were 17 children, ages 3 months through 4, in the care of 3 staff. Three children and three staff records were reviewed and updated. No medication is being administered at this time.

Areas observed were: outside playground, emergency supplies, hand washing/ bathroom and diapering procedures, lunch, and nap. Interactions between children and staff were observed as appropriate and the children seem aware of the daily routine and schedule.

Areas of non-compliance with licensing standards may be found on the violation notice. Please complete the corrective plan of action and return to the licensing inspector within 5 business days.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on record review, the facility failed to have two staff complete an updated tuberculosis screening.

Evidence:

1. Three staff records were reviewed.
2. Staff #1 and Staff #2 did not have documentation of obtaining an updated tuberculosis screening within 2 years of the prior one.
3. Staff #1 last documented TB screening was dated 06/18/2020. Staff #2 last documented TB screening was dated 03/25/2022.
4. Staff confirmed updated TB screenings had not been obtained.

Plan of Correction: Staff are in the process of obtaining a new TB screening and will email new results to our inspector.

Standard #: 8VAC20-780-280-B
Description: Based on observation the facility failed ensure hazard substances were kept in a locked place.

Evidence:

1. Once can of Febreeze and one bottle of sanitizing spray was observed on a shelf behind the toilet used by the children.
2.The director confirmed the hazardous substances were not locked.

Plan of Correction: Director removed and trashed Febreeze and put sanitizing bottle in a locked container.

Standard #: 8VAC20-780-500-A
Description: Based on observation, the facility failed to have six children wash hands after lunch.

Evidence:

1. Six children were observed eating lunch.
2. Six children were observed not washing their hands after eating lunch.
3. The teacher confirmed children did not wash hands after eating lunch.

Plan of Correction: Director spoke with staff about hand washing before and after each meal. Children and staff must wash hands.

Standard #: 8VAC20-780-560-F-4
Description: Evidence: Based on observation, one child was served a food that is considered to be a potential choking hazard.

1. One 2-year child was observed eating lunch that was brought from home.
2. The lunch consisted of a whole hotdog.
3. Staff did not cut the hotdog into bite size pieces.
4. Staff confirmed she did not cut the hotdog into bite size pieces.

Plan of Correction: Director spoke with the parent and staff. Any food that enters the building must be cut in to small pieces etc... hotdog, grapes, strawberries.
