An unannounced, on-site monitoring inspection was initiated on 6/4/2024 and the inspection concluded on 6/4/2024. The on-site inspection began at 10:39AM and ended at 11:41AM. The inspector reviewed compliance in the areas listed above. There were 10 children, 2 caregivers and 1 household member present. The inspector reviewed 3 children?s records and 1 household member record on-site on 6/4/2024. This inspection included document review, tour of the facility and observations.
Discussion on standards related to follow up tuberculosis screenings.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.036-B-2 Description: Household members 18 years of age and older must have a criminal history records check by the 30th day of residence. Household member 2 did not have criminal history record check results during the inspection on 6/4/2024. Plan of Correction: Provider will obtain criminal history check on household member.
Standard #: 8VAC20-770-40-D-4 Description: Household members 18 years of age and older must have a sworn statement completed prior to the first day of residence. Household Member 2 did not have a sworn statement completed during the inspection on 6/4/2024. Plan of Correction: Provider will obtain sworn statement from household member.
Standard #: 8VAC20-800-100-A Description: The provider shall obtain documentation of a physical examination prior to a child?s attendance or within 30 days after the first day of attendance. There was no physical exam record available for Child 1 who was provided care in June 2024. Child 1 has been enrolled for 4 months. Plan of Correction: Provider will obtain physical record.
