1. The wooden lattice barrier covering the air conditioning unit on the playground is broken. There is also exposed torn wire mesh with pointy sharp edges under the lattice barrier.
2. The center director confirmed that the wooden lattice barrier coving the air conditioning unit on the playground is not in a safe and operable condition.Plan of Correction: The center responded with the following: The wooden lattice barrier for the air conditioning unit will be repaired as soon as possible.
Standard #: 8VAC20-780-540-E Description: Based on a review of emergency supplies and interview, the center did not ensure that one working, battery-operated flashlight and one working, battery-operated radio is kept at the center.
Evidence:
1. The center does not have a battery-operated flashlight and does not have batteries for the battery-operated radio.
2. The center director confirmed that the center does not have the required battery-operated flashlight or batteries for the battery-operated radio.Plan of Correction: The center responded with the following: The flashlight for the center was mistakenly taken to the other center. The flashlight will be returned to the center today and batteries for the radio will be purchased.
