Comments:
A renewal inspection was initiated on April 23, 2024 from 10:45am to 12:30pm. There were nine children present, including one of the provider?s children. The children ranged in ages 2 to 5 years old and counted for a total of 16 points. The inspector reviewed children and household records, including a pet. Children were observed during play activities and music and movement. Required postings, emergency supplies, and emergency evacuation drills were also reviewed. The provider reports that children are transported, and that medication is not administered at the family day home. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit meeting.

Description: Based on observation the provider did not ensure that temperatures are maintained at or below 0?F in the freezer.
Evidence:
1.The thermometer in the freezer had a reading of 4?F.

Standard #: 8VAC20-800-210-A
Description: Based on record review and interviews, it was determined the provider did not ensure that in addition to satisfactory completion of first aid training and CPR training,
caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
1. For the year of 2023 caregiver #2 had only 3 of the 16 hours of annual training hours required.

Description: Based on observation the provider did not ensure that areas of the family day home, inside and outside, are maintained in a safe condition.
Evidence:
1.In the outdoor patio area, there was splintered and deteriorating wood, peeling paint and a nail protruding on a windowpane.
2. In the yard where children play, there was Poison Ivy growing at the base and on the tree trunk.
3. In the kitchen foyer, there was a light switch cover that was cracked, posing as a pinch hazard.

Description: Based on observation and interview the provider did not ensure that potentially poisonous substances be stored in areas inaccessible to children and away from food.
Evidence:
1.There were cleaning agents such as disinfectant wipes and air refresher on top of the cubby area in the main playroom.
a.The manufacturer?s labels read ?Keep Out of Reach of Children.?
2.There was a multi-purpose cleaner, witch hazel, and hydrogen peroxide stored in a low cabinet, in the bathroom.
a.Children were observed entering and exiting the bathroom.
3.There were two nonprescription medications on a shelf in the kitchen foyer.
4.A kitchen drawer contained a lighter and Gorilla glue.
5.On a kitchen countertop, there were cleaning agents such as air freshener, disinfectant cleaner, and bleach.
a. The manufacturer?s labels read ?Keep Out of Reach of Children? on the cleaning agents.
b. The bleach was stored next to a bottle of mustard.
6.The provider confirmed that the materials listed were accessible to the children in care.

Description: Based on observation the provider did not ensure that protective covers be installed on all unused electrical outlets.
Evidence:
1.There were three electrical outlets that were missing covers in the family day home.
