An unannounced complaint inspection was conducted on 05/20/2024 based on a complaint received on 05/10/2024 regarding neglect. Staff were interviewed and 5 staff records were reviewed. The inspector arrived for the inspection at approximately 10:30 am and departed approximately 11:30 am.
The information gathered during the inspection determined the complaint to be not valid.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations unrelated to the complaint and were documented on the violation notice issued to the provider.

Standard #: 8VAC20-780-70 Complaint related: No Description: Based on record review and interviews with staff, the center failed to ensure that staff records contain all of the elements as required by the standards.
Evidence:
1.The record for Staff 1 documented a hire date of 02/15/2024. The record for Staff 1 did not contain the following: documentation that two or more references as to character and reputation as well as competency were checked before employment. Staff 6 confirmed this information was not available. Staff 1 was observed providing direct care and supervision to children during the inspection.

The record for Staff 4 did not contain the following: documentation that two or more references as to character and reputation as well as competency were checked before employment. Staff 6 confirmed this information was not available. Staff 4 was observed providing direct care and supervision to children during the inspection.Plan of Correction: Provider will obtain.
Standard #: 8VAC20-780-340-F Complaint related: No Description: Based on observations and staff interviews, the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence:
At approximately 11:12 am on 05/20/2024, the inspector observed a child, verified to be 3 years of age, exit the classroom being supervised by Staff 7, enter Restroom A, out of actual sight and sound supervision of program staff. Restroom A is the first restroom on the left, approximately 20 feet and around a corner from the classroom door. Approximately, one minute later, the inspector notified Staff 6 of the child?s presence in the restroom.Plan of Correction: Provider will remind staff of sight and sound requirement.
