Violations:
Standard #: 8VAC20-780-270-A
Description: Repeat Violation
Based on observation the facility did not maintain the center, inside and outside, in a
clean, safe and operable condition.
Evidence:
*The playground fence, near the CH 1 room, had:
1. A support pole with broken brackets and missing bolts.
2. The end cap is missing on the cross bar at the top of the fence posing a finger
entrapment area.
* The CH 2 classroom had sharp objects accessible to children. An apple corer/slicer, a large, serrated edge plastic knife, and a "chopper" with a crimped metal edge were on a low shelf for use by children.

Plan of Correction: Brackets that were not in use due to missing bolts and having been replaced with wire ties or pipe straps, have been removed.
Remaining brackets now have bolts.
Poles with open ends have been capped with extra caps or liberally covered with electrical tape to impede access.
We removed sharp objects such as apple
slicers, choppers, and similar items from free access by children. Instead, we provide them to children under the supervision an adult nearby.
