An unannounced, on-site monitoring inspection was initiated on July 19, 2024 and completed on same, as a part of the denial process. The on-site inspection began at 8:47AM and ended at 10:15AM. The inspector reviewed compliance in the areas listed above. There were 27 children present and 6 staff. The inspector reviewed 2 children?s records and 1 staff record on-site. This inspection included:
document review, tour of the facility, interviews, observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Sarah.Marbert@doe.virginia.gov.

Standard #: 22.1-289.035-B-3 Description: Based on a review of records, one staff record did not contain documentation of a Central Registry search having been submitted prior to employment.
Evidence:
Staff A's record contained documentation of a submitted Central Registry Search (CRS) that was submitted on 7/10/24. (Date of Hire:7/9/24)Plan of Correction: CRS will be emailed to new employee with onboarding tasks with follow up completed 1 business day prior to first day.
