There were 44 children present, ranging in ages from six years to eleven years, with six staff supervising. Children were celebrating the last day of the program with a glow party. Children played games, made slime, face painted and had trets. Children also participated in a volleyball lesson and a fitness program.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and
supervision, programming, medication, special care and emergencies and nutrition. A total of five child records were reviewed. Five staff records were reviewed remotely on May 17, 2024. Violations:
Standard #: 8VAC20-780-540-E
Description: Based on observation and interview, the center did not maintain a working battery-operated radio in each building used by children.

Evidence:

The battery-operated radio on site did not work. The director stated she did not have any additional batteries for replacement.

Plan of Correction: Per the director: I will get new batteries for the radio and change them out.

Standard #: 8VAC20-780-550-P
Description: Based on review of documentation and interview, the center did not maintain a written record of children?s serious and minor injuries in which all required entries were included.

Evidence:

1. Nine out of twelve injury records reviewed did not contain the required time and method how the parent was notified of the injury. The record also did not contain staff and parent signatures or two staff signatures.
2. The director acknowledged the injury reports were missing the documentation and stated the injury record forms were unintentionally printed without the second page that contained the missing information. The three reports that were complete contained a second page with the information.

Plan of Correction: Per the director: I will reprint the injury forms and make sure both pages are attached.

Standard #: 8VAC20-780-560-F
Description: Based on observation and interview, the center did not ensure a menu listing foods to be served for meals and snacks for a one-week period was current.

Evidence:

The menu posted on the license board was dated April 2024. There was no current menu posted. The director stated she had not obtained the May menu from the school yet.

Plan of Correction: Per the director: I will get a copy of the menu from the cafeteria.
