Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable). A monitoring inspection was initiated on 08/13/2021 and concluded on 08/17/2021. The licensee was contacted by telephone to initiate the inspection. There was 1 child present and 1 caregiver. The inspector emailed the licensee a list of items required to complete the inspection. The Inspector reviewed 1 child records and 1 caregiver records along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-710
Description: Based on a record review and interview the licensee did not ensure all medication administration requirements were met.
Evidence:
Child #1's had an authorization to administer medication form in the record. The caregiver stated she gives medication on as "as needed basis." The caregiver confirmed she does not have Medication Administration Training (MAT).

Plan of Correction: The following corrective action was provided during the exit interview:
Medication will not be administered and all forms will be changed to reflect that.

Standard #: 22VAC40-111-250-C
Description: Based on observation the licensee did not ensure there were no hanging, suffocation and/or strangulation hazards.
Evidence:
There was a crib used for a 1 year-old child in care, that had a mobile tied across the top of the crib.

Plan of Correction: The following corrective action was provided during the exit interview:
The mobile was removed from the crib.

Standard #: 22VAC40-111-320-B
Description: Based on observation the licensee did not ensure child-resistant protective covers were over all electrical outlets.
Evidence:
In the kitchen there were several electrical outlets and a power strip that did not have protective covers.

Plan of Correction: The following corrective action was provided during the exit interview:
Covers will be kept in the kitchen and placed over all uncovered electrical outlets.

Standard #: 22VAC40-111-520-E
Description: Based on observation the licensee did not ensure all requirements when using cribs.
Evidence:

There was a crib used for a 1 year-old child in care, that had crib bumper pads attached to the slats.

Plan of Correction: The following corrective action was provided during the exit interview:
The bumper pads were removed from the crib.
