Comments:
The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 25, 2022 from approximately 1:30pm to 3:00pm. There were seven children present, ranging in ages from six months to four years, with two staff supervising.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-560-A
Description: Based on review of five staff records, the vendor did not ensure three staff members obtained a negative tuberculosis (TB) screening within the required timeframe.
Evidence:
1. The record for Staff #4 (Date of employment 10/10/2022) contained documentation of a TB screening dated 11/02/2020.
2. The record for Staff #2 (Date of employment 08/08/2022) contained documentation of a TB screening dated 06/27/2022.
3. The record for Staff #3 (Date of employment 08/29/2022) contained documentation of a TB screening dated 09/22/2021.
4. Staff are required to obtain a negative TB screening within 30 days prior to beginning employment.

Plan of Correction: Going forward, we will ensure all staff obtain a TB test within 30 days prior to being hired.

Standard #: 8VAC20-790-640-A
Description: Based on observation, the vendor did not ensure all hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevented access by children.
Evidence:
1. A can of germicide was sitting on the top of a shelf in the bathroom that was observed being used by children during the inspection.

Plan of Correction: The cleaning agent was moved to a locked location while the licensing inspector was on site.

Standard #: 8VAC20-790-820-B
Description: Based on observation, the vendor did not ensure when a child is placed in an infant seat, high chair, or feeding table, the protective belt was fastened securely.

Evidence:
During the inspection, the licensing inspector observed Child #1 sitting in a high chair eating. The high chair contained a protective belt. The protective belt was not buckled around Child #1.

Plan of Correction: The child was buckled in the high chair while the licensing inspector was on site. We will ensure all children are fastened in when sitting in the highchair.
