The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 24, 2023 from approximately 11:30 AM to 1:45 PM. There were 9 children in attendance and a total of two staff supervising. A total of five children and four staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-790-550-2-f Description: Based on review of four staff records and staff interview, the vendor did not obtain results of a sex offender registry check from any state in which one staff member resided in the preceding five years prior to beginning employment.
Evidence:
1. The record for Staff #3 (Date of Employment 10/16/2023) indicated Staff #3 lived in another state outside of Virginia in the past five years.
2. Staff #3 was present at the center caring for children.
3. The director confirmed the information during the inspection.Plan of Correction: The out of state background checks were requested while the licensing inspector was onsite.
Standard #: 8VAC20-790-550-5 Description: Based on review of four staff records and staff interview, the vendor did not ensure required records were kept for each staff person.
Evidence:
1. The record for Staff #4 (Date of Employment 08/01/2023) did not contain documentation of orientation.
2. A member of management stated the orientation had been completed but could not be located.Plan of Correction: We will complete the orientation form again. Going forward, we will ensure all paperwork is file in staff records.
