An unannounced monitoring inspection was conducted on September 12, 2023 from approximately 12:30 pm to 1:45 pm. Provider and two assistants were present with 9 children. Children were observed during naptime. Two child and three caregiver records were reviewed. Violations were found and are noted on the violation notice.

Standard #: 8VAC20-800-180-A Description: Based on review, provider did not ensure caregivers have TB tests or screenings every 2 years.
Evidence: The following was found during review of caregiver records:
1. The record for caregiver #1 had a most recent TB test/screening dated 7/8/2021 which is more than 2 years old.
2. The record for caregiver #2 had a most recent TB test/screening dated 8/5/2021 which is more than 2 years old.Plan of Correction: Not available online. Contact Inspector for more information.
