Comments:
An unannounced renewal inspection was conducted on July 2, 2024 from approximately 11:00 am to 2:00 pm. There were 17 children in attendance and a total of 4 staff present. 4 staff records and 18 children?s records were viewed. All areas of the center were observed including classrooms and bathrooms. The children were observed playing with toys on the floor, eating lunch, and taking a nap, while the licensing inspector was on-site.

Evacuation drills were conducted monthly with the most recent conducted on 06/23/24.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 07/12/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description: Based on record review, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. The record for child # 2 age 16 months, date of enrollment 08/16/2023, contained immunizations dated 07/17/2023. The record did not contain additional immunizations.
2. The record for child #3, age 22 months, date of enrollment 03/01/2023, contained immunizations dated 02/13/2023. The record did not contain additional immunizations.
3. The record for child #8, age 15 months, date of enrollment 07/24/2023, contained immunizations dated 06/21/2023. The record did not contain additional immunizations.
4. The record for child #10, age 2 years, date of enrollment 03/01/2022, contained immunizations dated 02/25/2022. The record did not contain additional immunizations.

Based on record review, and interview, the center failed to obtain a physical examination for each child within 30 days after the first day of attendance.
Evidence:

Based on record review, the center failed to maintain and keep at the center a separate record for each child that contains all required information.
Evidence:
1. 1 out of 18 children's records viewed did not contain the name, address, and phone number of two designated people to call in an emergency.
2. 2 out of 18 children's records viewed did not contain the names of persons authorized to pick up the child.
3. 11 out of 18 children's records viewed did not contain the previous child day care and schools attended by the child.
4. 6 out of 18 children's records viewed did not contain information on whether the child has allergies or intolerances to medication or any other substances, and actions to be taken in an emergency situation.

Description: Repeat Violation
Based on documentation review and observation, the center failed to obtain approval from the health department for food service.
Evidence:
1. The center does not have approval from the Health Department for food service. The center was informed by the Health Department on 01/26/2024 that only prepackaged foods that do not require heating and do not produce dishes are approved to be served.
2. The center was observed cooking fish sticks and french fries in an air fryer, and serving the food to the children for lunch.

Description: Repeat Violation
Based on observation, the center failed to ensure that children's hands shall be washed with soap and running water before and after eating meals or snacks.
Evidence:
1. The children were observed playing on the floor in the classrooms. The children were directed to get up off the floor and walk directly to the lunch table to eat. The children were not observed washing their hands before eating lunch. When the children were done eating several of the children were observed leaving the lunch table walking back to the classroom and laying down on their cots for nap. The children did not wash their hands after eating lunch.

Based on observation, and interview, the center failed to post a current dated menu for the one-week period.
Evidence:
There was not a menu posted for the current week. A handwritten note indicating what was being served on that day was posted. Administration confirmed that a current one-week menu was not posted.
