Comments:
An unannounced renewal inspection was conducted today from 10:30am-11:30am. There were 5
children directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 1 pet record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, directly with caregivers, and napping. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-800-750-A
Description: Based on observation, topical ointments present in the family day home did not meet all requirements.
Evidence:
Four of five topical ointments observed were not labeled with the child's name.

Plan of Correction: The provider corrected this violation by adding taped labels to all ointments. Provider use tape labels to avoid children's names from rubbing off in the future.

Standard #: 8VAC20-800-120-B
Description: Based on record review, assistant records did not contain all required information.
Evidence:
The record for Adult 2 did not contain: the employee's job title, proof of identity, date of employment or volunteering, and the name, address and phone number of a person to contact in an emergency.

Plan of Correction: The provider corrected this violation by adding adult 2 assistant provider record to her file.

Standard #: 8VAC20-800-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1. Child 2's record did not contain addresses for the two listed emergency contacts.
2. Child 5's record did not contain the child's health insurance policy number.

Plan of Correction: The provider corrected this violation by having both parents add the missing information to their child's paperwork.

Standard #: 8VAC20-800-200-A
Description: Based on record review, the provider did not orient the assistant caregiver by the end of their first week of assuming job responsibilities.
Evidence:
Adult 2's record did not contain documentation that an orientation was received.

Plan of Correction: Adult 2 had 10hr & 3hr Better Kid Care training prior to starting. The provider corrected this violation adding by adults 2 certificates to her file as well as her orient training from.

Standard #: 8VAC20-800-280
Description: Based on observation, potentially poisonous substances were not kept in an area inaccessible to children.
Evidence:
An aerosol disinfectant was observed in the bathroom cabinet. The cabinet is outfitted with a child lick that was not engaged.

Plan of Correction: The provider corrected this violation by engaging the child lock after every use.
