Comments:
An initial study was conducted on 04/05/2022 from 10:30 a.m. - 12:00 p.m. The family day home was observed for age/stage appropriate supplies, activities, and equipment for children aged birth - 12 years. Policies and procedures, emergency procedures, and emergency equipment for were observed. There were no children in care during the inspection. The provider and 3 household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-40-C-1-c
Description: Based on review of two household member records it was determined that not all household member records contained documentation of a central registry background check upon application for licensure. Evidence: During the announced initial inspection central registry background checks were not on file for household member A and household member B.

Plan of Correction: The provider will resubmit central registry background checks for household member A and household member B.

Standard #: 8VAC20-800-760-A-1
Description: Based on review of the first aid kit it was determined that not all required supplies were available. Evidence: The first aid kit did not contain triangular bandages.

Plan of Correction: The provider will purchase triangular bandages for the first aid kit.

Standard #: 8VAC20-800-770
Description: Based on interview and observation of the family day home it was determined that emergency flashlights and a weather band radio were not available to caregivers at all time. Evidence: The provider has not yet obtained a flashlight and weather band radio.

Plan of Correction: The provider will purchase a flashlight and weather band radio for the family day home.

Standard #: 8VAC20-800-790
Description: Based on observation of the family day home it was determined that required telephone numbers were not posted. Evidence: The telephone numbers for 911 or local police, fire, emergency medical responders, regional poison control, and the person responsible for emergency backup care were not posted.

Plan of Correction: The provider will post the required telephone numbers.

Description: Based on interview and observation of the family day home it was determined that an operable landline telephone was not available at the family day home. Evidence: The provider has not requested a landline telephone number or purchased a landline telephone for emergencies.

Plan of Correction: The provider will request a landline telephone line and purchase a landline telephone.

Standard #: 8VAC20-800-430-B
Description: Based on observation it was determined that pressure mounted or accordion expansion gates were not excluded. Evidence: A pressure mounted gate was observed at the stair opening.

Plan of Correction: The provider will purchase a wall mounted gate for the stair opening.
