Comments:
A subsidy monitoring inspection was conducted today, 8/9/2022. A Voluntary Registration inspection was also conducted on today's date. Those requirements for providers who are subsidy vendors were reviewed. There were two children in care with the caregiver. Two children's records and three provider/household records were reviewed.

Violations were cited and can be found on the Violations page of the inspection. The violations were reviewed with the provider and plans of correction obtained during the exit interview.

Violations:
Standard #: 8VAC20-790-190-B
Description: Based upon review of two children's records, the provider has not ensured that the children's records include the names, addresses and telephone numbers of persons to be contacted in an emergency if a parent cannot be reached.
Evidence:
The record provided for child 1 did not include emergency contact persons.

Plan of Correction: The provider responded with the following:
Al parents will be required to provide the names, address and telephone number of emergency contacts.

Standard #: 8VAC20-790-260-K
Description: Based upon observation, the provider has not ensured that stairs are not accessible to children less than two years of age and children older than two years of age who are not developmentally ready to climb or descend stairs.
Evidence:
The safety gate in the room where child 1 was napping was not latched during the inspection, making the stairway to the second floor potentially accessible to the child as there is no safety gate at the actual stairway.

Plan of Correction: The provider responded with the following:
The safety gates will be kept closed at all times when the rooms are being occupied by children.

Standard #: 8VAC20-790-390-F
Description: Based upon observation, the provider has not ensured that soiled disposable diapers are disposed in a leak proof storage system that is not hand operated.
Evidence:
The step-on diaper disposable can was located in a different room than the diaper changing table and thus was not ready for use as a diaper disposal system.

Plan of Correction: The provider responded with the following:
The diaper pail will be kept beside the changing table at all times.

Standard #: 8VAC20-790-420-A
Description: Based upon the review of the home's emergency plans with the provider, the provider has not ensured that the plan includes written shelter-in-place and lockdown procedures.
Evidence:
The provider stated that the emergency plan did not include written procedures for sheltering-in-place or a potential lockdown scenario

Plan of Correction: The provider responded with the following:
Shelter-in-place procedures will be added to the emergency preparedness plan.

Standard #: 8VAC20-790-430-A-2
Description: Based upon provider interview, the provider has not ensured that shelter-in-place drills are practiced twice per year.
Evidence:
The provide stated that shelter-in-place drills had not been practiced.

Plan of Correction: The provider responded with the following;
Shelter-in-place procedures will be practiced and documented twice per year.

Standard #: 8VAC20-790-430-A-3
Description: Based upon provider interview, the provider has not ensured that a lockdown drill is practiced one time per year.
Evidence:
The provide stated that a lockdown drill had not been practiced.

Plan of Correction: The provider responded with the following:
Lockdown procedures will be practiced and documented once annually.

Standard #: 8VAC20-790-440-C
Description: Based upon provider interview, the provider has not ensured that a menu of foods being served for a one week period is posted.
Evidence:
The provider sated that she had not posted the menu for this week.

Plan of Correction: The provider responded with the following;
The menu will be posted on Monday of each week.
