Comments:
An unannounced monitoring inspection was started in the center on April 23, 2024 from 12:00 to 2:00 PM and concluded on April 25, 2024. At the time of entrance,49 children were in care with 6 staff members. The sample size consisted of 6 children?s records and 6 staff records. Children were observed reading books, dancing and preparing for and eating lunch.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A
Description: Based on record review and staff interviews, the licensee did not ensure to obtain documentation that each child has received a physical examination by or under the direction of a physician before the child's attendance; or within 30 days after the first day of attendance.

Evidence: The following records did not include physicals
1.Child #2, first date of attendance 9-11-2023
2.Child #6 first date of attendance 10-3-2023

Plan of Correction: Child #2 - The Health Coordinator received Physical examination documents for child #2 on 04/23/2024. Family Services made a copy and placed it in the child's classroom folder on 04/24/2024.

Child #6- The Health Coordinator followed up with the mom on 04/23/2024 regarding the physical examination documents. The mother of child # 6 has informed the Family Services Specialist that the child has an appointment on 04/29/2024 and will turn in the documentation the next day. The Health Coordinator and Family Services will follow up with the mother on 04/30/2024.

Standard #: 8VAC20-780-70
Description: Based on record review and staff interviews, the licensee did not ensure the required staff information to be kept at the center for staff records contained all required documentation.

Evidence: The record for Staff #5, did not include information about any health problems that may interfere with fulfilling the job responsibilities or their emergency contact information.

Plan of Correction: Staff #5 was filling in at the site on the date of inspection. A copy of their emergency contact form was sent to the site on 04/23/2024 after the inspection was completed.

Human Resources provides a copy of all emergency contact forms upon hiring to each Site Supervisor, and the forms are placed in a notebook to which all staff have access. Copies are also provided to the Education Manager and Transportation Coordinator.
