Technical Assistance:
A review of center?s safe sleeping practices determined that these practices are not consistent with ?The Three A?s? safety precautions. A recommendation was made to revise the practices to be consistent with ?The Three A?s? precautions identified in the ?Safe Sleep 365? flyer, which was provided to the center at the time of inspection and July 31, 2023.

Comments:
The licensing inspector initiated an unannounced inspection on 05/14/2024 in response to a complaint received by the department on 04/24/2024 for allegations regarding hazardous substances. Interviews and observations were conducted relating to the allegations. The information gathered during the investigation does not support the allegation, so the complaint could not be substantiated.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
Please return a signed and dated copy of each page of this notice to the licensing inspector no later than 5:00pm on 05/31/2024. If you?d like to include a plan or correction and date to be corrected for each violation cited, please include that information with the signed and dated copy of the Violation Notice by the deadline. Plans of correction should include steps to correct the noncompliance with the standard and measures to prevent the noncompliance from occurring again.

Description: (REPEAT VIOLATION) Based on observation and a review of the center's Policy and Procedure for Safe Sleep and SUIDS Prevention, the licensee failed to ensure that the center maintained in compliance with the center's own policies and procedures that are required by the standards.

Evidence:

A crib in the older infant room (all infants under 12 months) had a very lose sheet covering the mattress. The center's Policy and Procedure for Safe Sleep states: 10. A safety-approved crib with a firm mattress and tight-fitting sheet will be used.
One of the cribs in the older infant room (all infants under 12 months) had a thick blanket. The center's Policy and Procedure for Safe Sleep states: 7. No pillows, comforters, bumper pads, etc. will be used in cribs.

Plan of Correction: New crib sheets have been ordered to ensure that all sheets available are tight fitting sheets. New sheets should arrive on June 10, 2024.
The thicker blanket was sent home on May 14, 2024. This was a new blanket that the child had never brought before this day. This blanket was for a child that would turn 1 year old in a week and was working on transitioning to a cot. This child also uses a blanket for soothing purposes.
Policy and procedures for Safe Sleep and SUIDS Prevention has been revised. An updated copy with more specific language has been sent to the licensing for review. Licensing approved the revised policy on May 23, 2024. Parents and staff received a copy of the revised policy on May 31, 2024.
No Blanket Zone signs have been placed on the wall near the cribs in the nursery and infant classrooms. Photo proof was submitted to licensing on May 21, 2024.
Blankets were sent home on May 15, 2024 and sleep sacks have been requested.

Description: (REPEAT VIOLAITON) Based on observation and interview, the center failed to ensure staff supervising children always provided care, protection, and guidance.

Evidence:

In the infant room, (all infants under 12 months) three infants were in cribs sleeping with blankets.
In the older infant room, (all infants under 12 months) six cribs had blankets in them. Staff stated infants were allowed to sleep with blankets.

Plan of Correction: No blanket Zone signs have been placed on the wall near the cribs in the nursery and infant classrooms. Photo proof was submitted to licensing on May 21, 2024.
Additional training in Safe Sleep was received by staff in the infant classrooms. Training certificates were submitted to licensing on May 20, 2024.
Blankets were sent home on May 15, 2024 and sleep sacks have been requested.
