Comments:
An unannounced non-mandated monitoring inspection was conducted on 03/14/2023. There were 10, ages 7 months-3 years, totaling 24 points, one substitute provider, and one assistant caregiver present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, physical equipment and environment, care of children, preventing the spread of disease, and nutrition. A total of five children?s records were reviewed. The children were observed during free choice play and having breakfast. The inspector arrived for the inspection at 9:30 am and departed at 10:40 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-800-960-C
Description: Based on observation, the provider failed to ensure that highchairs with safety waist and crotch straps are fastened according to the manufacturer's instructions.

Evidence: Child 1 was observed in a highchair during breakfast. The safety straps of the highchair were not fastened.

Plan of Correction: Will review with the staff.

Standard #: 8VAC20-800-60-B
Description: REPEAT VIOLATION

Based on record review, the provider failed to ensure that children?s records shall contain all of the elements as required by the standards.

Evidence:
1. The record for Child 1 did not contain the following information: the office address of the child?s physician; name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (there was only one designated contact person listed); first date of attendance; and documentation of the review of the child?s emergency contact information.
2. The record for Child 2 did not contain the following information: name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (there were no addresses provided for the two designated contacts).
3. The record for Child 5 did not contain the following information: the office address and telephone number of the child?s physician; names of persons other than the custodial parents who are authorized to pick up the child; and the first date of attendance.

Plan of Correction: Will review every child's records and get with parents.

Standard #: 8VAC20-800-680-A
Description: Based on observations and interviews, the provider failed to ensure that caregivers shall wash their hands with liquid soap and warm running water before feeding or helping children with feedings and after diapering a child.

Evidence:
1. Caregiver 2 was observed feeding Child 2 a bottle. Caregiver 2 did not wash their hands before feeding the child a bottle.
2. Caregiver 2 was observed during diapering. Caregiver 2 did not washing their hands after diapering. Caregiver 2 verified they did not washed their hands with soap and warm running water after diapering a child.

Plan of Correction: Will review with the staff.

Standard #: 8VAC20-800-680-B
Description: REPEAT VIOLATION

Based on observation, the provider failed to ensure that children?s hands are washed with liquid soap and warm running water after toileting or diapering.

Evidence: Caregiver 2 was observed during a diaper change. The caregiver did not ensure that the child?s hands were washed with liquid soap and warm running water after the diaper change.

Plan of Correction: Will review with the staff.

Standard #: 8VAC20-800-690-D
Description: Based on observation and interviews, the provider failed to ensure that the diapering surface shall be cleaned and sanitized after each use.

Evidence: Caregiver 2 did not sanitize the diapering surface after changing a child?s diaper. Caregiver 2 verified that the diapering surface was not sanitized after the diaper change.

Plan of Correction: Will review with the staff.
