Comments:
An unannounced focused monitoring inspection was conducted on 07/12/2023. There were 9 non-resident children and 1 resident child, ages 6 months-3 years, totaling 26 points, one substitute caregiver and one assistant caregiver present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, physical equipment and environment, care of children, preventing the spread of disease, and nutrition. A total of 5 children?s records were reviewed. The children were observed during breakfast and in free choice activities. The inspector arrived for the inspection at 9:15 am and departed at 10:30 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-800-900-B
Description: REPEAT VIOLATION

Based on observation, the provider failed to ensure that children shall not be allowed to eat or drink while walking, running, playing, lying down or riding in vehicles.

Evidence:
1. Child 4 was observed on a bouncing toy playing while drinking from a cup.
2. Child 5 was observed drinking from a cup while walking around.
3. Child 6 was observed drinking from a cup and eating a cereal/granola bar while walking around.

Plan of Correction: Will have a conversation with the assistant caregivers.

Standard #: 8VAC20-800-320-B
Description: Based on observation, the provider failed to ensure that child-resistant covers shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

Evidence: There was an electrical outlet located on the wall behind the crib closest to the couch that had been covered with electrical tape to prevent use and access. The electrical tape had been peeled up so that the top outlet could be used, an orange extension cord was plugged into the top outlet, and the bottom outlet was uncovered which exposed the outlet. The outlet was visible and accessible through the slats of the crib.

Plan of Correction: Will have the assistants cover the outlet.
