An unannounced monitoring inspection was conducted on 05/08/2024. There were 10 children and 1 resident child, ages 10 months-4 years, totaling 26 points, and three caregivers present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, physical equipment and environment, and care of children. The children were observed having breakfast, participating in learning activities, and in indoor large motor play. The inspector arrived for the inspection at 9:45 am and departed at 10:55 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-770-60-B Description: Based on record review, the provider failed to ensure that a caregiver must not be employed until the agency has the person?s completed sworn statement or affirmation.
Evidence: The record for Caregiver 3 contained a documented hire date of 04/22/2024. There was not a sworn statement in the record for Caregiver 3.Plan of Correction: A sworn statement was completed during the inspection.
Standard #: 8VAC20-800-100-A Description: Based on record review, the provider failed to obtain documentation of a physical examination prior to attendance or within 30 days after the first day of attendance.
Evidence: The record for Child 3 contained a documented start date of 04/01/2024. There was not documentation of a physical examination in the record for Child 3.Plan of Correction: The parent was contacted to request the information during the inspection.
