Technical Assistance:
The following were discussed with provider:

1- 8VAC20-780-440 J. There shall be at least:
1. Twelve inches of space between the sides and ends of occupied cribs except where they touch
the wall; and
2. Thirty inches of space between service sides of occupied cribs and other furniture where that
space is the walkway for staff to gain access to any occupied crib.

2- 8VAC20-780-340.
F. Children under 10 years of age always shall be within actual sight and sound supervision of
staff. Discussed staff positioning in the classroom.

3- Discussed the difference between medication authorization form and allergy action plan from the physician.

4- Resilient surfacing - Had a discussion with the center to determine which type of resilient surfacing to use on the playground . The resilent surfacing selected need to be at least 6inches.

Comments:
An unannounced monitoring inspection was conducted on May 20, 2024 from 10:00am- 12:40 pm. The inspection concluded on May 20, 2024. At the time of entrance, 66 children were in care with 13 staff members present. Additional staff was present to assist. The sample size consisted of 7children records and 6 staff records. Children were freely playing, being diapered, outside playing, preparing to go outside and participating station activities. Outside areas were inspected during the inspection.

Standard #: 22.1-289.035-B-4
Description: Based on record review and staff interview, the licensee did not ensure to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
-Staff #3 lived in Pennsylvania in the past 5 years.
-Staff #3 hire date was 2/12/2024.
-Staff # 3 record did not obtain criminal record check results for Pennsylvania
- Staff #3 criminal record should be obtain completed before reporting to child day center.

- Staff #4 lived in Alaska in there past five years.
- Staff #4 hire date was 2/5/2024.
- Staff #4 record did not obtain criminal record before reporting to the child
day care center.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the licensee did not ensure hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In a unlock women's bathroom in the hallway, there was three cans of aerosol spray and one container of disinfectant wipes. The hazard substances were labeled with Keep out of reach of children and Caution.

In the infant classroom, there was a bottle of white out that have labels that states" Danger" and " Keep out of reach of children."
