An unannounced, on-site focused inspection was conducted on 5/3/24 as part of an Intensive Plan of Correction. The on-site inspection began at 8:50 am to 11:35 am. The inspection reviewed compliance in the areas of background checks, allergy compliance, staff files and training. Twenty-two staff files were reviewed as well as the posted allergy list, allergy care plans and interviews with staff regarding allergies/sensitivities/dietary restrictions in each classroom.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Description: Repeat Violation:
A copy of the criminal history record information is required at the time of employment from some states in which the individual has resided in the preceding five years and out of state central registry background checks must be requested by the end of the 30th day of employment.
There was no out of state criminal record check on file for staff #1, who resided in Washington DC within in the last five years.
The center has not requested from Washington D.C. the out of state central registry background checks within 30 days of employment. Staff #1 has been employed for seven weeks.Plan of Correction: We will get the out of state background checks.
Standard #: 8VAC20-770-60-C-2 Description: Repeat Violation:
Central Registry findings are required within 30 days of employment Staff A was hired 16 months ago and there was no central registry finding on file.Plan of Correction: I will send off for and obtain this background check.
Standard #: 8VAC20-780-240-C Description: Repeat Violation:
Orientation training did not include all of the following topics within seven days of assuming job responsibilities.
Staff A through M were missing orientation training that addressed emergency preparedness and response planning to include violence at a child care facility, safe sleep practices, abusive head trauma, coping with crying babies or distraught children, prevention and response to emergencies due to food or other allergic reactions, recognizing symptoms of allergic reaction and preventing the exposure. Staff A through M have been employed at the center from 10 days to 22 months.Plan of Correction: I will use the form on the public website in order to cover all areas of orientation training.
