Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on May 1, 2024 regarding allegations of Behavioral Guidance. The inspection was initiated on May 15, 2024 and completed on May 20, 2024. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.

The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations found during the inspection regarding *. See the violation notice for more details.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Description: Based on record review and interview, the center did not have documentation of central registry findings for one staff member.

Evidence:
1. The record for staff #1 (employed: 10/31/2023) did not contain documentation of central registry findings until 01/02/2024.
2. Administrative staff confirmed that the central registry finding were not obtained by the end of the 30th day of employment. No documentation was recorded that provider contacted the central registry office regarding the delay in the findings.
