Comments:

A Subsidy Health and Safety inspection was conducted on 2/8/2024 in conjunction with a code compliance inspection. A total of 18 children were present with 5 caregivers. The children were engaged in the following: singing and dancing, learning about black history month, learning shapes, and coloring rainbow crafts. Programming, menus, emergency procedures, and 5 children?s records and 4 staff records were reviewed. There were reportedly no medications on site. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 3/25/2024. Description:	Based on record review, the vendor did not obtain the results of an out of state criminal history background check prior to hire or request a search of the child abuse and neglect central registry within 30 days of employment for all states in which staff have lived within the previous 5 years. Evidence: Staff #3 (Date of Hire: 8/30/2023) has lived in the states of Rhode Island and North Carolina within the last 5 years. Staff #3 did not have a Rhode Island criminal history background check on file. Staff #3's record was also missing child abuse and neglect central registry search requests for North Carolina and Rhode Island.
