An unannounced monitoring inspection, resulting from a self-reported incident, was conducted on 05/15/2024. The inspector reviewed video footage, interviewed 1 staff, and reviewed a total of 1 staff record and 1 child?s record. The inspector arrived at 11:15 am and departed at 2:40 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-780-340-A Description: Based on video footage and interview with the staff, the center failed to ensure that when staff are supervising children, they shall always ensure their care, protection and guidance.
Evidence: Video footage from 05/02/2024 shows Child 1 (13 years old) sitting on a mat in the middle of the room. Staff 1 is seen walking towards Child 1 pointing their finger at the child. Child 1 is seen kicking at Staff 1, slapping Staff 1 in the face, and then throwing themselves back onto the mat. Staff 1 is seen holding Child 1?s hands to stop the hitting as Child 1 continues to kick at Staff 1. Staff 1 then stands up, places her foot underneath the mat and rolls Child 1 off of the mat onto the floor. Child 1 can been seen sitting themselves back up. Staff 1 did not provide the proper care, protection and guidance to Child 1.Plan of Correction: The staff member was terminated effective 5/3/24 after the incident occurred.
