Comments:
An unannounced monitoring inspection was conducted on 03/13/24 from 8:20 a.m. to 10:00 a.m. with the provider. There were seven children in care, ranging in age from 12 months to seven -years-old, for a total of 13 points. Children were observed eating breakfast, playing with manipulatives, and watching an educational program. Six child records and one staff record were reviewed. The provider has current certification in CPR and First Aid. The first aid kit, emergency drills, and attendance were also reviewed. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 8VAC20-790-250-B
Description: Based on observation and interview, it was determined that the provider did not ensure that children be protected from materials that could be swallowed or present a choking hazard. Toys or objects less that 1-1/4 inches in diameter and less that two inches in length shall be kept out of reach of children under the age of three years old.

Evidence: A small toy was observed near a 12-month-old child, the toy was approximately 1 inch in diameter and 1-1/2 inch in length, posing a choke hazard.

Plan of Correction: Toy was removed during inspection. Did a thorough check to ensure there were no more small toys on toddler side.

Standard #: 8VAC20-800-780-B
Description: Based on review of five children's records, the provider did not ensure that they reviewed with the parent the emergency contact information annually for each child and obtained the parent's signature acknowledging the review.

Evidence: The last documented annual review for Child #1 was 12/03/22, Child #3 was 11/30/22, Child #4 was 11/14/23, and Child #3 was 11/14/22.

Plan of Correction: All parents reviewed and updated records on 04/21/24.

Description: Repeat Violation

Based on documentation review, the provider did not ensure that wood burning fireplaces and associated chimneys were inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed.

Evidence: The most recent fireplace inspection was dated 01/09/23.

Plan of Correction: Fireplace Inspection scheduled for 04/05/24.

Standard #: 8VAC20-800-460-I
Description: Based on observation the provider did not ensure litter boxes were inaccessible to children.

Evidence: A litter box with animal feces was observed on the kitchen floor approximately four feet away from where the children were seated and eating breakfast.

Plan of Correction: New puppy had just used box as I was moving kids from kitchen to daycare downstairs as you walked in. Litter box is always cleaned out A.S.A.P. but has been moved to another area.
