Technical Assistance:
Consultation provided on the two playgrounds, written policies, procedures, parent and employee handbooks.

Comments:
An announced initial inspection was conducted today with the director and administrative staff.
The spaces inside and outside were inspected as the space had previously been measured and supports the requested capacity. The classrooms were appropriately furnished and had ample supplies on site or supplies that have been ordered for the requested age range. The infant cribs were manufactured in March 2012. The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, it was determined that the center was not maintained in a safe and operable condition.
Evidence:
The platforms and steps on the multicolored climbing and slide structure on the preschool playground was severely decaying, rusting and cracking.

Plan of Correction: We will place the play structure off limits with signage, tape and by training the teachers to make sure the structure wont be used until we can repair the platform and steps.

Standard #: 8VAC20-780-330-B
Description: Based on observation and measurements, it was determined that there was inadequate resilient surface surrounding play equipment on the preschool playground.
Evidence:
The resilient surface ( poured-in-place) on the preschool playground surrounding the multicolored composite climb and slide structure was cracking, separating and had divots in the surface therefore was no longer considered an adequate resilient surface.

Plan of Correction: We will place the structure off limits with signage and tape and train teachers to make sure the play structure is not used until we are able to repair the platform, steps and poured in place.
