Comments:
An unannounced monitoring inspection was initiated and concluded on October 4, 2023. There were two children in care ages 4 years and 18 months old with the provider. Children present totaled five points. Children were observed in provider and self-directed activities. Children and household records were reviewed. Emergency equipment, evacuation drills, first aid kit and required postings were reviewed. Violations regarding children's records and required postings were observed, discussed with the provider and appear on the violation notice.

Description: Based on observation and interview the provider failed to post required documents
Evidence:
1 - The most recent violation notice is not posted in the family day.
2 - The provider confirmed that the most recent violation is not posted in the family day home.

Description: Based on record review and interview the provider failed to ensure that children's records contain all required information.
Evidence:
1 - Upon review the record for Child #2 did not contain the required information, including mother's name and employment contact, doctor's telephone number and address, emergency contact telephone number, medical insurance policy number, substitute care agreement.
2 - The provider confirmed that the record of Child #2 did not contain the information listed.

Standard #: 8VAC20-800-70-A
Description: Based on record review the provider failed to ensure that before the child's first day of attendance parents are provided in writing whether or not there is liability insurance of at least in force on the family day home.
Evidence:
1 - The record for Child #1 does not contain any documentation that the parents were informed of the status of family day home's liability insurance.
2 - The provider confirmed that the documentation was not in the record of Child #1.
