Comments:
A monitoring inspection was conducted on May 1, 2024 from 10:30 A.M.-2:00 P.M. There were 45 children present, ranging in ages from three months to five years of age, with eleven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and eight staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-140-B
Description: Based on a review of five child records and interview, the center did not ensure that if a child has had a physical examination prior to attendance, it was within the required time period.

Evidence:
1. The record of child #2 (age 14 months at the time of enrollment 04/08/2024) contained a physical dated 6/13/23. Physicals are required to be completed within three months prior to attendance for children seven months-18 months of age.
2. The record of Child #3 (age three years at the time of enrollment 04/26/2024) contained a physical dated 03/20/2023. Physicals are required to be completed within 12 months prior to attendance for children two years of age through five years of age.
2. Administration acknowledged that the physical did not occur within the required time frame.

Plan of Correction: Management will check dates on physicals and ensure that the date is in the required time per state standards.

Standard #: 8VAC20-780-70
Description: Based on a review of staff records, the center failed to ensure that documentation of orientation training was completed in the staff record.

Evidence:
1. Staff #1's record did not contain documentation of orientation training being completed. Staff #1's date of hire was 04/08/2024.
2. Administration verified that the documentation for orientation was not completed in the staff record.

Plan of Correction: Management will make sure to check off each subject covered in the orientation process as it is completed. Documentation will be placed in the employee file.

Standard #: 8VAC20-780-420-E-1
Description: Based on review of infant reports, the center failed to ensure that the records contained all the required information.

Evidence:
1. Child #1's, (age 3 months), daily record did not contain documentation of tummy time for 04/30/2024.
2. Staff #2 verified that tummy time is not documented at all times.

Plan of Correction: Staff were reminded of the proper record keeping procedures for tummy time and it pertains to any infant who can not roll over. Administration will check periodically to ensure that tummy time is being documented.

Standard #: 8VAC20-780-520-A
Description: Based on review of over-the-counter skins products, the center failed to ensure that over-the-counter skin products shall be kept beyond the expiration date.

Evidence:
1. In the Three Year Old Classroom there was the following expired over-the counter skin products; Mineral Sunscreen (expired 10/2021), Desitin Diaper Ointment (expired 02/2022), and Studio Select Sport Sunscreen (expired 12/2022.)
2. In the Four Year Old Classroom there was a tube of Studio Selection Sunscreen for Kids that expired in 10/2023.
3. In the Two Year Old Classroom there was a tube of Aeevno Baby Eczema Therapy Cream that expired in August 2023.

Plan of Correction: Staff were told to examine all over-the-counter skin products and ensure that the dates are not expired. Staff were told to discard any out of date products and to check the date before accepting them from parents.

Standard #: 8VAC20-780-550-P
Description: REPEAT VIOLATION
Based on a review of injury reports, the center failed to ensure that all required information was in the injury reports.

Evidence:
1. On an injury report dated 04/01/2024, and three injury reports dated 04/02/2024,the time the parents were notified was not completed.
2. On one injury report dated 04/02/2024 how the parent was notified was not completed.
3. Administration verified that the injury reports were missing required information.

Plan of Correction: Administration reviewed the process of documenting injury reports with staff. Employees were remined of the required information needed on the reports and shown what was insufficient on their completed reports.
