Comments:

An unannounced monitoring inspection was conducted today from 8:00am-9:30am. At the time of arrival, there were 3 children present directly supervised by the provider and 1 assistant caregiver. The home, 2 caregiver records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely. Areas of non-compliance are identified in the Violation Notice. Description:	Based on record review, the provider did not Review with the parent the emergency contact information required in 8VAC20-800-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review. Evidence: 1. The most recent documentation of emergency contact information in the record for Child #2 was dated 5/19/2023. 2. The most recent documentation of emergency contact information in the record for Child #3 was dated 5/16/2023.
