An unannounced subsidy health and safety inspection was conducted on 5/23/2024 beginning at 2:00pm and ending at 2:45pm. There were 2 children in attendance at the time of the inspection. Required background searches, training certificates and household member information was reviewed. A sample of requirements for daily attendance, child information/records, emergency preparedness planning and supplies were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the vendor. Standard #: 8VAC20-790-190-B Description: Based on review of children's information, it was determined that written authorization for emergency medical care and parent work phone numbers were not documented.
Evidence: There was no written authorization for emergency medical care for Child #1 and Child #2. There was no phone number for the parent of Child #1.Plan of Correction: Vendor will obtain emergency medical authorization for both children and work phone numbers for the parent of Child #1.
