Comments:
An unannounced monitoring inspection was initiated 6/3/2024 and completed on 6/11/2024 . The inspector was on site from 3:30pm until 5:20pm.

There were 30 children, ages 5 to 11 years old under the supervision of two staff. Children were observed during indoor free play and snack time. Five children's records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.011-F
Description: REPEAT VIOLATION
Based on observation, the center failed to ensure that the license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

Evidence:
1.The license was posted on the freezer door of the refrigerator in the small kitchen area and was not posted in a conspicuous place on the licensed premises.
2. The license posted was expired, staff personal could not find the current license to post.

Plan of Correction: The license was found in the Coordinators bag and posted 6-4.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
In the main playroom there was an exposed socket. The cover for the socket was missing exposing the plugs in the wall. This is a hazard to children. Children were observed in the area.

Plan of Correction: Maintenance was called, and the outlet was fixed 6-21

Description: Based on observation, the center failed to
ensure that hazardous substances,
shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1.In kitchen area there was insulation foam and caulk under the sink. This cabinet had no locking mechanism on it. Children were observed in this area.

Plan of Correction: The insulation foam and caulk were thrown out 6-20

Description: Based on observation, the center failed to ensure children?s hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.

Evidence:
Children were observed eating snack on the carpet by the tv. After the children finished there was no handwashing or wipes given to wipe hands.

Plan of Correction: The Program Specialist will review this with the Coordinator/teachers 7-1

Standard #: 8VAC20-780-560-M
Description: Based on observation, the center failed to ensure that no child shall be allowed to drink or eat while walking around.

Evidence: Children were observed having snack on the carpet. There were children walking around with bowls of food eating during this time.

Plan of Correction: The Program Specialist will review this with the Coordinator/teachers 7-1
