Comments:
An Initial inspection was conducted today. The playgrounds and rooms were measured to calculate capacity for the facility.
The emergency preparedness plan was submitted to the county for review. The updated plan will be reviewed by the Licensing Inspector (LI).
Room supplies and equipment, playground equipment, first aid supplies and emergency supplies are all in compliance.
Violations reviewed with the Head of School: background search documentation, TB result documentation, lead teacher qualifications and training as part of orientation. Model forms were provided for emergency evacuations, staff file cover sheet and training log requirements (emailed).
LI was present to day from10:00 AM - 1:00PM.

Description: Based on a review of records, staff records did not contain the results of a nationwide fingerprint search prior to employment.
Evidence:
5 of 9 staff files reviewed did not contain documentation of fingerprint results prior to employment.
*Staff #2 - results were dated 07/12/23. Date of Hire (DOH): 05/22/23.
*Staff #3 - results were dated 06/16/23. DOH: 06/05/23.

Description: Based on a review of records, one staff record did not contain documentation of a criminal history check for an out-of-state residence within the past five years.
Evidence:
Staff #3's record did not contain documentation of a criminal history check for a residence in another state within the past 5 years, prior to employment. DOH: 06/05/23.

Description: Based on a review of records, one staff record did not contain documentation of a completed Sworn Disclosure Statement (SDS) prior to employment.

Staff #9's completed SDS on file was dated 06/20/2023. Date of Hire (DOH): 06/19/2023.

Plan of Correction: Staff's completed Sworn Disclosure Statement (SDS) was on file by 6/20/23.
Moving forward, all new employees will have the SDS on file prior to their Date of Hire.

Description: Based on a review of records, one staff record did not contain documentation of all of the orientation requirements within 7 days of the date of assuming job responsibilities.
Evidence:
Staff #1's record did not contain documentation of Child Abuse and Neglect training as required by ? 63.2-1509.

Plan of Correction: Staff completed the corrected version of the training. The results are now on file.
Moving forward, all new employees will complete the training within 7 days of employment.
