8VAC20-790 Subsidy Regulations.

Comments:
An unannounced initial Subsidy Health and Safety Inspection (SHSI) was conducted on 3/18/24 from 11:05am to 1:30pm with the Director. There were 37 children in care, ranging in age from six-months to five-years-old, supervised by eight staff. Children were observed playing ?Duck, duck, goose!? and basketball in the gym; listening to a story; having free play; and using the restroom and washing hands to get ready for lunch. Five staff records and five child records were reviewed for required documentation. Two medications and their authorizations were reviewed. There are children currently enrolled with diagnosed food allergies and their records were reviewed for allergy care plans. The last evacuation drill was documented on 3/11/24. One classroom was observed for supplies for school-age children as the program wants to begin offering a Summer Camp. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 3/25/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-790-560-A
Description: Based on review of five staff records, the program did not ensure that staff were evaluated by a health professional and issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Evidence: The records for Staff #1, Staff #2, Staff #3, and Staff #5 did not contain documentation of a TB screening.

Plan of Correction: Health documents were placed in staff's folders during Inspector's visit. Moving forward Director will keep better log of placing documents in folders. TB screenings have been scheduled, and Director will place record in staff folders when completed.

Standard #: 8VAC20-790-640-A
Description: Based on observation, the program did not ensure that hazardous substances such as cleaning materials, were kept in a locked place using a safe locking method that prevented access by children. Evidence: In each of the five classrooms the program is currently using, hazardous substances including disinfecting wipes and sprays, were observed to be on countertops and high shelves, but not locked.

Plan of Correction: During visit, Director moved all hazardous materials and substances to locked cabinets. In one classroom, the locks will be re-keyed so we are able to lock cleaning products.

Standard #: 8VAC20-790-670-H
Description: Based on observation, the program did not ensure that children less than 10 years of age were always within actual sight and sound supervision of staff. Evidence: 1. At approximately 11:40am on 3/18/24, a ten-month-old infant was observed napping alone in a classroom. The teacher who was responsible for the supervision of the child was separated from the child by a floor-to-ceiling wall with a doorway between the two rooms that had a baby gate. 2. At approximately 12:10pm on 3/18/24, two four- to five-year-old children were observed alone in their classroom while the rest of the class and two staff were down the hall using the restroom.

Plan of Correction: 1. Director moved a teacher into the doorway of the classroom, so the teacher had proper supervision and was within sight/sound of the child napping.
2. Director moved the 2 children from the room, and brought them into the hallway with their class. One parent was notified regarding the child's unwillingness to remain safe, and stay with their group. Director spoke to both teachers and reminded them that they needed to have supervision at all times. Teachers will use walkie-talkies to ensure safety of all students.

Standard #: 8VAC20-790-770-A-2
Description: Based on review of two medications, the program did not ensure that prescription medication for a child was dispensed from a pharmacy and maintained in the original, labeled container. Evidence: A prescription emergency medication was observed on-site for Child A. The medication was in a bag and there was no box or prescription label with the medication.

Plan of Correction: On 3/18/24 Director requested original prescription medication box from parents in writing. Note was placed in child's folder.
