Comments:
An unannounced monitoring inspection was initiated on July 8,2024 and completed on July 15, 2024 in response to a self-report received by the licensing office on July 3, 2024 relating to Administration and Staffing / Supervision. This inspection included document review, interviews and observation. This inspection was amended on July 24, 2024 to adjust risk ratings.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 23, 2024. A POC submitted after this date will not appear on the public website

Violations:
Standard #: 8VAC20-780-40-E
Description: Based on center documentation and staff interviews, the licensee did not ensure, the operational responsibilities of the licensee shall include ensuring the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

Evidence: Staff #1 and Staff #2 did not follow the center's supervision policy and procedures to include "Never leave a child unattended." and "Staff members must be able to see and hear the children in their care and be readily available for (potential) emergencies at all times." Child #1 was left alone on the playground for approximately 30 minutes on July 2, 2024.

Plan of Correction: In addition to retraining, we have ensured each staff member that was involved does not work alone with children. We have also initiated extra checks before and after children go outside where administration is helping with the face to name checks and doing playground sweeps when they come back in.

Standard #: 8VAC20-780-340-A
Description: Based on center documentation and staff interviews, the licensee did not ensure when staff are supervising children, they shall always ensure their care, protection, and guidance.

Evidence: On July 2, 2024, from approximately 5:00pm to 5:30pm, Child #1, (three years old) was alone outside on the center?s playground. Staff #1 and Staff #2 were unaware Child #1 was on the playground until Child #1?s parent arrived for pick up at approximately 5:30pm. Staff #3 and Child #1?s parent found Child #1 on the playground. According to weather underground the temperature was 80 F.

Plan of Correction: Center Management has already put several things in place to ensure this never happens again. The director has retrained the entire staff on our supervision policy, which includes a face to name training. She has also retrained on our Name to Face Policy. We have updated our Internet so that the iPads for the name to face procedure works smoothly and properly outside.

Standard #: 8VAC20-780-340-F
Description: Based on center documentation and staff interviews, the licensee did not ensure children under 10 years of age always shall be within actual sight and sound supervision of staff.

Evidence: On July 2, 2024, from approximately 5:00pm to 5:30pm, Child #1 (three years old) was not within actual sight and sound supervision of staff, as the child was alone outside on the center's playground.

Plan of Correction: As a center, we have also initiated extra checks before and after children go outside where administration is helping with the face to name checks and doing playground sweeps when they come back in.
