Comments:
An unannounced renewal inspection was conducted today from 12:30pm-1:10pm. There were 7 children directly supervised by 3 caregivers. The home, 3 caregiver records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Areas of non-compliance are identified in the Violation Notice.


Please submit your plan of correction within five business days in order for it to be included on the public website.

Violations:
Standard #: 8VAC20-800-100-A
Description: THe provider is required to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Two children's records (Child #1 and Child #2) did not contain documentation of a physical examination.

Plan of Correction: The files were completed will all the required documents. Parents emailed me the missing paperwork. Milenka is the person that will monitor that there are no missing documents in their files.

Standard #: 8VAC20-800-40-A
Description: THe provider shall ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of six children. On the date of inspection seven children were receiving care in the home.

Plan of Correction: THe number of children will be reduced to 6 I have informed the parent we can't exceed the licensed capacity. It was an unexpected event and will not happen again. Milenka will be responsible to make sure that there are only 6 children.

Standard #: 8VAC20-800-70-A
Description: Before children's first date of attendance the provider is required to inform parents, in writing, of specific policies, procedures, and information. Three children's records (Child #1, Child #2, Child #3) did not contain documentation that parents have been informed of all required information.

Plan of Correction: Parents were informed of policies and have signed the forms. Milenka is the person that will monitor that there are no missing documents in their files.

Standard #: 8VAC20-800-80-B
Description: The provider is required to verify the age and identity of children in care. Three children's records (Child #1, Child #2, Child #3) did not contain documentation that the children's age and identity were verified.

Plan of Correction: Signed and dated when Birth Certificates were shown to the Program Representative. Milenka is the person that will monitor that there are no missing documents in their files.

Standard #: 8VAC20-800-90-A
Description: THe provider is responsible for obtaining documentation that children have been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations prior to attendance. Two children's records (Child #1 and Child #2) did not contain documentation of immunizations.

Plan of Correction: All the files were updated and now have the immunization records on file. Milenka is the person that will monitor that there are no missing documents in their files.
