Comments:
An announced initial inspection was conducted Wednesday, January 3, 2024. The licensing inspector was assisted during the inspection by the director. The program plans to provide care for children birth through six years in age. The program will provide care from 7:00 am to 6:00 pm Monday thru Friday. The facility can accommodate the requested capacity of 174 children. The center has a sensory room, and ten classrooms. Outdoor space includes an infant play area, and two fenced playground areas. The center is equipped with toys, educational materials, games, furnishing, arts and crafts materials and other items for the children to use. All areas of the facility were inspected.
Policy and procedures were submitted to the department and reviewed. Transportation will not be provided at this time. Medication will be administered with proper parent authorization when needed and parents will provide meals and the center will provide snacks.
During the inspection, the inspector reviewed compliance in the areas of administration, physical plant, staffing, medication, special care and emergencies and nutrition. Compliance with some standards could not be determined since the center is not yet in operation. A total of two staff records were reviewed along with background checks for the board officers.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. The center is in substantial compliance with the standards that could be determined. A six-month conditional license will be recommended. During the conditional period, the center will be subject to unannounced licensing inspections, with the first inspection taking place within the first sixty days of licensure.

Standard #: 22.1-289.035-B-2
Description: Based on a review of two staff records and interview, the center did not ensure one staff
member completed a national fingerprint check prior to the first day of employment and within
the required time frames.

Evidence:

1. The national fingerprint check for Staff #1, employed 09/11/2023, was dated 09/12/2023
exceeding the first day of employment.
2. The director acknowledged the fingerprints background check was not completed prior to employment.

Plan of Correction: Per the director: Going forward, all fingerprint background checks will be completed prior to hire.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of two staff records and interview, the center did not obtain the results
of the central registry background report within 30 days of employment for one staff member.

Evidence:

1. The record of staff #1, employed 09/11/2023, did not contain the results of a central
Registry background report exceeding 30 days from the date of employment.
2. The director stated he did not have a completed central registry check for staff #1.

Plan of Correction: Per the director: Central registry checks will be requested at the time of employment and results will be obtained with 30 days of employment.

Standard #: 8VAC20-780-160-A
Description: Based on review of two staff records, the center did not obtain documentation of a negative
tuberculosis screening at the time of employment for one staff member. The documentation shall have been completed within the last 30 calendar days of the date of employment.

Evidence:

1. The record of staff #1, employed 09/11/2023, contained documentation of a
tuberculosis screening dated 10/07/2023, exceeding 30days prior to employment.
2. The record of staff #2, employed 12/11/2023, did not contain documentation of a tuberculosis screening exceeding the first day of employment.
3. The director acknowledged the record of staff #2 did not contain documentation of a tuberculosis screening.

Plan of Correction: Per the director: We will ensure staff obtain a negative tuberculosis screen prior to employment.
