Comments:

An unannounced non-mandated inspection was conducted on May 20, 2024 between 2:55pm and 3:45pm. There are no children enrolled at the time of this inspection. The inspector reviewed compliance in the areas of administration, required posted documents, physical plant, household members files, and health related services. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Based on record review, and interview, the Sworn Statement was not completed prior to the 1st day of residence. Evidence: 1. Household Member #1 did not have a signed sworn statement. and did not have a central registry results. 2. Household Member #2 did not have a signed Sworn Statement and did not have a central registry results. 3. Household Member #3 did not have a signed Sworn Statement and did not have a central registry results. 4. Household Member #4 did not have a signed Sworn Statement and did not have a central registry results. 5. The provider acknowledged none of the household members had a signed sworn statement nor a central registry results on file.
Plan of Correction:	I thought the criminal history search was all I needed. The household members are moving out by 5/27/24. I haven't had any children enrolled but I will be sure to get background checks for all household members in the future.
Standard #:	8VAC20-800-180-A
Description:	Based on record review and interview, the provider failed to obtain for each adult household member a current Report of Tuberculosis Screening form. Evidence: 1. Household Member #1 did not have a current TB screening on file. 2. Household Member #2 did not have a current TB screening on file. 3. Household Member #3 did not have a current TB screening on file. 4. Household Member #4 did not have a current TB screening on file. 5. The provider acknowledged the household members did not have a current TB screening on file.
Plan of Correction:	I did not have children enrolled and they were moving out before the children would start in June. I will be sure to get TB screening in the future for household members.
