Standard 8VAC20-800-920 regarding the posting of the menu was discussed with the provider.
- Comments:

A monitoring inspection was initiated and concluded 03/20/2024. The inspector arrived at 12:30 PM and departed at 2:12 PM. There were 3 children present, totaling 6 points, with the provider and an assistant supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 provider/assistant records were reviewed.
Information gathered during the inspection determined a non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program. Violations were reviewed with the provider during the exit interview.

Standard #: 8VAC20-800-60-B Description: Based upon review of children's records, the provider has not ensured that each child's record includes the name, address and telephone number of each parent who has custody.
Evidence:
The record provided for child 3 did not include the name, address and telephone number for the father. No custody papers were in the record.Plan of Correction: The provider responded with the following:
