Comments:
An initial inspection began and concluded on January 10, 2024. The childcare provider was present with one nonresident child in care. Total points present were 4 points. The child in care was observed during rest and snack time. The entire home and outside play area were reviewed. One child record and two adult caregiver/household records were reviewed. Additional required records, postings, and emergency supplies were also reviewed. Violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection. A six-month conditional license is recommended to allow the applicant the opportunity to demonstrate continued compliance with the Standards for Licensed Family Day Homes. All future inspections will be unannounced. The first unannounced inspection will be conducted within 60 days from the date the license is issued. Violations:
Standard #: 8VAC20-800-60-B
Description: Based on record review and interview, it was determined that the provider did not ensure that children?s record contain all the required information.
Evidence:
The record for child #1 did not contain the date of enrollment, parent work phone number, name and policy number of the child?s medical insurance, and the results of a health examination.

Plan of Correction: The parent was contacted and the record was updated to include all required information.

Standard #: 8VAC20-800-350-E
Description: Based on observation, it was determined the provider did not ensure that hot water at taps available to children were maintained within a range of 105?F to 120?F.
Evidence:
When the temperature of the tap water in the bathroom was measured with a thermometer, it registered as 124 degrees.

Plan of Correction: The temperature of the hot water heater was turned down and now registers as 116 degrees.

Standard #: 8VAC20-800-430-B
Description: Based on observation and interview, the provider did not ensure that accordion expansion gates and pressure mounted gates shall not be used as protective barriers at stair openings.
Evidence:
There was a pressure mounted expansion gate at the bottom of the stairs leading to the second floor.

Plan of Correction: The pressure gate was removed and a wall mounted gate was installed.
