Comments:
An unannounced monitoring inspection was conducted on 6/7/2024. This family day home is on a conditional license. Two inspectors were on site from approximately 8:58 am-9:56 am. There were 0 children in care, and 1 caregiver. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, care of children, emergencies, caregiver training, preventing the spread of disease and nutrition. A total of 4 child records, and 1 caregiver record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-800-920
Description: Based on observation and interview, the provider did not ensure that when meals or snacks are provided by the family day home, the menu for the current one-week period must 1. Be dated; 2. Be given to parents or posted or placed in an area accessible to parents; 3. List any substituted food; and 4. Be kept on file one week at the family day home.

Evidence:
1. A dated weekly menu was not available at the time of the inspection.
2. The provider stated that it was not available.

Standard #: 8VAC20-800-60-A
Description: Based on a review of four child records and interview, the provider did not ensure to maintain a complete record for two children.

Evidence:
1. The record of child #1 (date of enrollment: 5/31/2024) was missing one emergency contact address. The record of child #4 (date of enrollment: 6/3/2024) was missing two emergency contact addresses. Child records are required to contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.
2. The provider acknowledged that the records were incomplete.

Description: Based on interview, the provider did not ensure that the diapering surface was separate from surfaces used for children's' activities, non absorbent and washable, and cleaned and sanitized after each use as required.

Evidence:
1. In interview, the provider stated that she regularly changes the children directly on the floor in a space that is used for children's activities to include play and rest.
2. The provider acknowledged that diapering on the floor did not meet the requirements.
