Comments:
An unannounced non-mandated monitoring inspection was conducted on June 18, 2024 from 9:57am until 11:00am. The director was not on-site during the inspection. The inspector reviewed compliance in the areas of general procedures, administration, physical plant, programs and staffing and supervision. A total of 2 children?s records and 3 staff records were reviewed and interviews were conducted.

The center is not in compliance of the consent agreement issued May 14, 2024.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on interviews and document review, the center failed to have a qualified program director or
a qualified back-up program director who meets one of the director qualifications who shall
regularly be on site at least 50% of the center?s hours of operation.
Evidence: 1. The center?s hours of operation are 6:30am-6:00pm open for a total of 57.5 hours a week.
2. A review of the program director?s attendance log from April 1, 2024-June 14, 2024 contains documentation of the following:
-The week of April 1-5; present on April 1st for 6 hours, April 2nd for 6.5 hours and April 5th for
2.25 hours. Total hours on-site for the week is 14.75 hours.
-The week of April 8-12; present on April 8th for 5 hours and not on-site for the remainder of
the week. Total hours on-site for the week is 5 hours.
-The week of April 15-19; present on April 16th for 5 hours and not on-site for the remainder of
the week. Total hours on-site for the week is 5 hours.
-The week of April 22-26; present on April 23rd for 5.5 hours and April 25 for 1.75 hours. Total
hours on-site for the week is 7.25 hours.
-The week of April 29-May 3; present on May 1st for 4 hours and not on-site for the remainder
of the week. Total hours on-site for the week is 4 hours.
-The week of May 6-10; present on May 7th signed in at 1:27pm and did not sign out. Not on-site for the remainder of the week. Total hours on-site for week is possibly 4.5 hours.
-The week of May 13-17; not signed in at all this week. Total hours on-site for the week is 0.
-The week of May 20-24; present May 21st for 6 hours and May 22nd for 5.5 hours. Total on-site for the week is 11.5 hours.
-The week of May 27-31; not signed in at all this week. Total hours on-site for the week is 0.
-The week of June 3-7; not signed in at all this week. Total hours on-site for the week is 0.
-The week of June 10-14; present June 11th signed in at 11:35am and didn?t sign out. Not on-site for the remainder of the week. Total on-site for the week is possibly 6.5 hours.
3. Two of four staff members interviewed reported the program director is on-site one day, every other week.
4. The program director was not on-site at inspections on June 18th and May 29, 2024.
