Comments:
A complaint was received by the licensing office on May 29, 2024, in regard to special feeding needs and staff qualifications and training. An investigation was initiated on May 31, 2024 between the hours of 10:25am - 1:05pm, and concluded on June 5, 2024. Observations were made, interviews were completed, and documentation was reviewed. The preponderance of evidence gathered supports violations of the standards. Areas of noncompliance can be found in the violation notice. Areas of noncompliance not related to the complaint but identified during the course of the investigation can also be found on the violation notice. Description: Based on record review, annual training relevant to staff's job responsibilities and the care of children was not completed by all staff.
1. Staff #1's (date of hire: 01/26/09) and Staff #4's (date of hire: 11/25/19) record's did not contain documentation of the prevention of shaken baby syndrome and abusive head trauma including procedures to cope with crying babies and distraught children.
2. Prevention of shaken baby syndrome and abusive head trauma including procedures to cope with crying babies and distraught children is relevant to Staff #1 and Staff #4's job responsibilities.

Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. In the Infant A classroom, 3 crib mattresses were cracked, exposing the foam on the inside.

Plan of Correction: Order new mattresses and replace the old ones.

Description: Based on observation, when an infant was placed in his crib, he was not placed on his back (spine).
Evidence:
1. Staff #2 placed Child #1 (age: 8 months) in their crib on their stomach.
2. Staff #2 acknowledged that they placed Child #1 in their crib on their stomach.

Description: REPEAT VIOLATION

Based on observation, a menu listing foods to be served for meals and snacks during the current one-week period was not dated.
Evidence:
1. The menu posted outside of the kitchen was not dated for the current one-week period.
2. In the Infant B classroom, child #1's (age: 8 months) menu was not dated for the current one-week period.

Plan of Correction: Update menu with correct dates and current meals.

Description: Based on record review, infants were not fed in accordance with parental instructions.
Evidence:
1. Child #1's (age: 8 months) parent provided instructions indicates that child #1 should be fed at approximately 7:30am, 10:30am, 1:30pm and 4:30pm.
2. Child #1 arrived on 05/28/2024 at approximately 9:06am. Child #1 took a nap from approximately 10:02am-12:12pm. Child #1 was fed their first bottle at approximately 1:15pm.

Plan of Correction: Ask parents for feeding schedule for formula bottles and post it in the room.
