Comments:
An unannounced inspection was initiated on 6/12/2024 in response to a complaint that was received by the licensing department on 6/7/2024 regarding allegations of inadequate supervision. On 6/12/2024, the inspector was on site from approximately 1:30 pm to 2:35 pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. Additional interviews were conducted remotely.

The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there was a violation found during the inspection regarding reporting. See the violation notice for more details.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on a review of documentation and interview, the center did not ensure to inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Evidence:
1. Child #1 (date of attendance: 2/5/2024) sustained an injury on 6/6/2024 that required outside medical attention. In interview, staff stated that they were made aware on 6/6/2024 that the child received medical care.
2. The department did not receive notification from the center within the two business days.
3. Administration acknowledged that the required notification to the department's representative was not made.

Plan of Correction: Managers will ensure that all incidents that require outside medical attention be reported within two business days. All staff have been retrained on this plan of action.
