Comments:

An unannounced monitoring inspection was conducted on May 15, 2024. Two licensing staff were on site from approximately 10:45am until 3:15pm. There were 63 children present in licensed care with 14 staff supervising. There was a total of six classrooms visited, ranging in ages from 18 months to school age. Children were observed in variety of activities such as singing and dancing, artwork, playing outdoors. Licensing staff interacted with staff and children in care. Staff interacted with children in a positive and caring manner. The inspectors reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 10 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #:	22.1-289.035-B-2
Description:	Based on a review of 10 staff records and interview, the center did not ensure one staff completed a national fingerprint check prior to the first day of employment and within the required time frames. Evidence: 1. The national fingerprint check for Staff #5, employed 8/15/2023, was dated 08/31/2023.
Plan of Correction:	Require fingerprint checks to be returned to school before any paid work occurs.
Standard #:	8VAC20-780-160-A
Description:	Based on review of 10 staff records, the center did not ensure two staff records had documentation of a tuberculosis screening that was completed within the last 30 calendar days of the date of employment and prior to coming into contact with children. Evidence: 1. The Tuberculosis documentation for staff #6 (date of hire 09/19/2022) was dated 08/15/2022. 2. The Tuberculosis documentation for staff #10 (date of hire 08/15/2023) was dated 08/18/2023.
Plan of Correction:	if TB documentation is older than 30 days, the TB screening will be repeated.
