Comments:
An unannounced inspection was initiated on May 1, 2024, and the inspection concluded on May 31, 2024, as a result of a self-report received in the licensing office on May 1, 2024. The inspection began at 10am and ended at 12:30pm. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 25, 2024. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-340-A
Description: When staff are supervising children, they shall always ensure their care, protection, and guidance. A 22 month old child was left alone on the playground for approximately 10 minutes without staff knowledge.

1. Staff #1 confirmed that she did not complete the name to face attendance when back in the classroom. Staff #1 stated ?I screwed up? and was accountable for what happened with leaving a 22 month old child alone on the playground.
2. Staff #2 stated she had no communication with Staff #1 and was unaware of the number of children that went inside the classroom and how many remained outside with her.
3. Staff #3 stated that she found Child #1 outside on the toddler playground alone without staff supervision when she took her preschool classroom outside. Staff #3 stated that it was at least 10 minutes that the toddler child was left unattended.
4. Staff #3 took Child #1 back to the classroom and the teachers and children were on the carpet doing story time. Staff #1 and Staff #2 had no knowledge a toddler child was missing from their classroom.

Plan of Correction: To provide great quality of care Center on face to name calling, CSR and ensure no child is left behind. Our policy states that demonstrating respect for children and sheltering them from risk is at the heart of everything we do. As a Company, we will show respect for children, provide a nurturing, enriching, and safe environment, and protect them at all times to the very best of our ability. All enrolled children must be supervised at all times. This means that employees must actively work to protect the safety of children and to ensure that children are protected from any dangers by intervening quickly if potentially dangerous situations arise. This may include a gentle and respectful physical intervention with a child when necessary to protect the child, other children, or employees from harm. In order to ensure that ALL classrooms are checking CSR's and face to name properly we will also do observations on transitions making sure staff are doing it correctly every time, stop signs has been added to every room to remind staff before transitioning the kids.

Standard #: 8VAC20-780-340-F
Description: Children under 10 years of age always shall be within actual sight and sound supervision of staff. A 22 month old child was left on the playground for approximately 10 minutes without staff supervision.

1. On May 1, 2024, Staff #1 took 5 of the 11 children (ages 16 months to 2 years of age) into the classroom from the playground. Staff #2 was left with 6 toddler children on the playground. Staff #1 confirmed that she did not complete the name to face count when Staff #2 came back into the building.
2. Staff #2 stated that she was unaware that Staff #1 went inside the classroom with children. Staff #2, unaware of how many children with her outside, took the remaining children into the classroom.
3. Staff #3 stated that she found Child #1 outside alone without staff supervision approximately 10 minutes after the toddler?s left their playground area. When Staff #3 brought Child #1 back to the classroom, Staff #1 and Staff #2 had no knowledge Child #1 was missing from their class.

Plan of Correction: Supervision of Children
Staff Members should minimize blind spots or potential hiding areas in all areas of the center.
Staff Members should intervene quickly when problems arise to help children work through problem solving and conflict resolution, while showing respect for children's development when addressing conflicts.

Standard #: 8VAC20-780-350-B-2
Description: In rooms with children ages 16 to 24 months, the required ratio is one staff per five children. There were 11 children supervised by 2 staff members.
1. Staff #1 stated there were 11 children in care with two staff members in the toddler room during the time of the incident where Child #1 was left on the playground unsupervised by staff.
2. The name to face attendance dated May 1, 2024, confirmed that the toddler room was over ratio with children to staff.

Plan of Correction: Center Director will retrain all staff members on ratios. CD and AD will be doing 4 checks throughout the day to make sure all classrooms are always maintaining ratio. If we have a teacher call out and are unable to maintain ratio either the CD or the AD will step in and fill there spots. We will also review the standards with teachers to ensure that they understand that keeping the ratio is important and that is the only way of keeping the children safe while in our care, We will also make sure the teachers understand that the CSR is a . proper tool used to accurately supervise and keep up with the children.
